Mr Anup, You can submit from 4th May onwards at the same branch. Documents required are Original Policy bond, a copy of PAN card, one cancelled cheque with policyholder's name printed on it. Please go to the branch with all these documents, They would give you the required forms. Fill and submit there itself.
